    Research Statement

     

    Pablo Yepes researchs in the area of heavy ion interactions. This includes the study of matter under extremes conditions of temperature and density, which may provide important clues for the understanding of the early universe. In addition these collisions have opened new possibilities in photon-photon collisions at large energies (photons are quanta of light). He has concentrated his efforts in the STAR experiment at the Relativistic Heavy Ion Collider at the Brookhaven National Laboratory. He played a major role in the design and development of fast analysis tools for the experiment. He has also made essential contributions in the analysis of Ultra-Peripheral Collisions. In addition he is part of a collaboration proposing the use of the next Large Hadronic Collider at CERN (European Laboratory for Particle Physics) in Geneva to study ion-ion interactions.     Improving Proton Therapy

     


     


    An important focus on my research in the last years has been in the area of medical physics. Since 2007, a fruitful collaboration with MD Anderson has been developed. This effort fits well with the university guidelines for the next century of fostering interdisciplinary research and collaboration with other institutions, especially with the Texas Medical Center.


     


    My efforts have been centered around proton therapy for cancer treatment. MD Anderson operates a Proton Therapy Center since 2006. This kind of radiotherapy has theoretical advantages over traditional radiotherapy with x-ray (photons) or electrons. The physical characteristics of the proton make it easier to minimize secondary effects, when treating patients with this type of radiation. Nevertheless, a number of issues need to be understood and controlled in order to take full advantage of the physical characteristics of proton radiation. One of those issues is how to calculate accurate dose distributions for a given set of radiation specifications. Proton dose distributions are more sensitive to the characteristics of the radiation and the environment than for traditional photon radiation. The most accurate distributions can be obtained with Monte Carlo methods. However, at the moment, they require long calculation times even with large computer resources. I have developed the Fast Dose Calculator (FDC), a track-repeating Monte Carlo approach, which drastically reduces the calculation times, without significantly compromising the accuracy of the results.


     


    There are several applications of FDC we are investigating and developing. I received funding from IBM to develop a web interface for FDC. The intention is to provide a universally accessible tool to quickly estimate the advantages of proton therapy over traditional therapy for particular patients. Even though the number of proton therapy centers in the US, and in the world, has grown considerably in the last few years, they are still a small percentage of the total radiation therapy facilities. However, there are cases where proton therapy clearly presents clinically advantages over traditional therapy. For example, pediatric patients are more at risk of secondary cancers, produced by the treatment itself, due to their longer life expectancy. A prototype website is online (fdcw.rice.edu). Further work is still needed to make it an universally usable tool.


     


    Since 2012, we have closely worked with our collaborators at MD Anderson (Dr. Radhe Mohan, Dr. Dragan Mirkovic, Dr. Uwe Titt, Dr. Sharmalee Randeniya) to make FDC available to researchers and clinicians at MD Anderson and study some cases of particular interest. We have focused on two areas: 4D lung cancer patients and IMPT.


     


    Some forms of lung cancer seems particularly appropriate for the techniques I have developed. Analytical dose calculation algorithms used in the clinic work less well in anatomic areas with large inhomogeneities, where very dense (bone) and light (lung) structures are spatially close, like the thorax. FDC can provide higher quality dose calculations in those areas. Preliminary studies seems to indicate that there are cases where the differences in the dose calculated with the analytical methods and Monte Carlo are not negligible. Moreover, there are some cases where a cancer recurrence occurred in a area that was well irradiated according to the analytical methods, but was not according to the Monte Carlo calculations. Such results, if confirmed, will indicate a strong need to improve dose calculations to optimize treatment plans.


     


    In addition, the problem of motion needs to be addressed when treating lung patients. The patients needs to breath during the treatment, what makes the tumor a moving target. The usual input for dose calculation is a 3D CT (Computer Tomography), which represents the patient as a 3D grid, where each element of the grid is assigned to a particular biological materials. For the thorax a 4D CT is used, where a series of 3D CTs are taken for various phases of the breathing cycle. At the moment, a 4D CT is used as input for the calculations, however it is turned into an “average” 3D CT, by taking for a given position of the 3D grid, the most dense material found there during the breathing cycle. FDC is fast enough that the dose distribution for each of the breathing cycles could be calculated separately, and then sum up properly according to the time the patient spends in each of the breathing phases.


     


    In addition we are applying FDC to IMPT (Intensity Modulated Proton Therapy), the most advance technique for proton therapy. This technique uses a plurality of very narrow proton beams to conform the radiation to the intended target. It is a very computationally intense technique because it requires the optimization of thousands of variables. An accurate calculation of dose distributions, as those provided by FDC, is particularly important for an accurate outcome.


     

     
    Application of Particle Physics Techniques to Radiation Physics
     

    Yepes started a collaboration with the University of Texas, MD Anderson Cancer Center in 2007. The goal of the collaboration is the development of a fast tool for patient treatment plan calculations using stochastic (Monte Carlo) techniques. Stochastic methods are more acurate than commonly used analytical approaches. However stochastic approaches are significantly more demanding in computational resources. The Fast Dose Calculator, developed as fruit of the collaboration, preserves the accuracy of stochastic approaches while reducing calculation times from hours to minutes.
